Sorry to hear that. Let’s start about your refrigerator….new? old? frost free? location? number of days aging? Was the meat in cryovac when you bought it – or out of a meat display?
My guess is it was either already contaminated, and/or the refrigerator failed to remove the moisture from the air as the meat aged and thee temperature was set too high or not maintained.
